The Position

Position Title: Care Coordinator Transition Care Program / GEM@Home
Classification: Grade 2 Allied Health professional or Grade 3 Registered Nurse
Salary per hour (exc. super): dependent on qualification
Hours per fortnight: up to 72hrs
Employment Status: fixed term

Key Responsibilities:
Provide support, care planning & care coordination after a client's hospital admission
Develop & foster collaborative relationships across the health service
Be the central point of contact for all involved in the care coordination process

The Team

The Integrated Care Services Division offers a diverse range of programs and services which provide high quality person centred care to patients, clients, residents and consumers in Bendigo and across the Loddon Mallee Region. Services are provided in inpatient, outpatient, community, home and residential care settings.

Our services include; Aboriginal Services and Diversity, Allied Health, Community Programs, Health Promotion, Outpatient Rehabilitation Service, Pastoral Care and Chaplaincy Services, Rehabilitation & Geriatric Medicine, Palliative Care, Residential Care Services, Transition Care and Volunteer Services.

In addition the Executive Director Integrated Care Services has professional responsibilities as the Chief Allied Health Officer As such the CAHO is focused on leading and supporting the allied health professional workforce to feel valued and empowered to provide exceptional, quality care.

Selection Criteria

Essential:
01.Relevant Allied Health or Nursing degree with associated registration.

Essential:
02.Commitment to ongoing professional development, and ability to meet relevant Bendigo Health Credentialing requirements (dependant upon qualification)

Desirable:
03.Demonstrated understanding of aged care and of the Transition Care Program, including the care coordinator role in this service

Desirable:
04.Clinical experience in care coordination / case management / discharge planning, with a focus on aged care or rehabilitation

Desirable:
05.Understanding of Aged Care Assessment Services and Aged Care legislation regarding residential placement

Desirable:
06.Demonstrated understanding of the impact of hospitalisation on older people

Desirable:
07.Demonstrated ability to work collaboratively as an individual practitioner within an interdisciplinary team environment including the ability to maintain effective relationships

Desirable:
08.Sound time management and prioritising skills

Desirable:
09.Excellent interpersonal, written and verbal communication skills, including excellent computer literacy.

10.A personal approach which is positive and friendly and works within the values of Bendigo Health
