About The Role

As Care Manager, you are responsible for ensuring a high standard of clinical care and nursing services are provided to residents within our residential aged care homes. You will be working as the 2IC or “2nd In Charge” to the Director of Care and Services, providing a high level of clinical support to the clinical services team. As an integrated village, you will need to work collaboratively and cooperatively with the independent retirement living village as well to ensure a positive resident experience for all.

You Will Be Responsible For

  • Facilitating the smooth entry of new residents into the aged care facility
  • Supporting residents and their families during departures from the aged care facility, including liaising with family members and solicitors
  • Ensuring resident care plans are reviewed at least 3 monthly b
  • Supporting the Director of Care and Services to manage the delivery of high quality clinical services, providing clinical leadership to the registered nursing and care service teams
  • Providing oversight of Standard 2, ensuring appropriate actions are taken, reviewing documentation and closing out any incident notes
  • Collaborating with residents and their families to ensure high levels of satisfaction, proactively resolving issues and facilitating family case conferences at least annually. Escalate issues to the Director of Care and Services where needed.

Successful Candidates Possess The Following
  • Previous experience working within a residential aged care facility in a similar role as Clinical Nurse or ACFI Coordinator
  • Completion of tertiary qualifications in nursing, healthcare, or similar (Bachelor of Nursing)
  • Current Registered Nurse registration with AHPRA
  • Thorough understanding of the Aged Care Legislation and demonstrated understanding of ACFI assessment processes
  • Comprehensive clinical knowledge and understanding of the ageing process, particularly age-related health issues
  • Strong coaching / training skills with the ability to positively influence other healthcare workers
