About the role

To provide person centred and contemporary community services to support healthy ageing, in line with Southern Cross Cares Better for Life model, for home care package clients in metropolitan Adelaide by:

  • Coordinating and providing clinical leadership to promote the well- being and independence of clients to live the best they can
  • Collaborating with clients and listening with empathy to their needs, goals and preferences
  • Supporting clients to make their own informed decisions to manage their day- to- day life to enable them to live as well as they can
  • Partnering with other health care team members and service providers for referral and care integration
  • Monitoring and evaluating services to ensure client satisfaction
  • Facilitating early identification and intervention for signs of decline
  • Providing leadership and support to the care team.

The successful candidate will have: 

  • Nursing or Allied Health degree with current registration with AHPRA as a Registered Nurse or Allied Health professional
  • Current driver’s licence and vehicle insurance
  • Previous experience in case management and care planning in an aged care environment
  • Previous experience working with CALD clients and groups including Aboriginal and Torres Strait Islander Australians
  • Experience in the use of community aged care systems and processes such as My Aged Care, ACAT, and RAS etc

All Southern Cross Care employees also require a current National Police Certificate for the Purpose of Aged Care.
