JOB DESCRIPTION

An exciting opportunity currently exists for an enthusiastic and experienced Allied health Clinician to work on a permanent full time basis (1.0 EFT) with the Chronic & Complex Care Division at Western Health as a Team Leader managing staff who work in the following services:



  • ACE inpatient Care Coordination

  • ACE ED Care Coordination

  • Rapid Allied Health (RAH) at home service

  • GEM @ Home pilot program


We are seeking applicants who are passionate about integrated care and patient flow and have experience and qualifications to lead change and ensure the delivery of person centred, evidenced based Best Care. The Team Leader will provide the day to day management of the care coordination staff in the ACE Team and Allied Health staff working in the at Home teams. The position will suit a person who is looking for a new challenge, enjoys working in a changing and fast paced environment and is committed to providing best patient care.


To be successful in this role we are looking for a leader with the following skills and experience:



  • Adaptability to work in an innovative and changing environment

  • Excellent communication and organisational skills

  • Experience in delivering results in specified timeframes

  • A commitment to the delivery of Best Care to our patients

  • Willingness to work across silos of care to deliver best care

  • Has held a Leadership role in a health related field

  • Previous experiencing in the management of staff working across a matrix arrangement

  • The management of complex rosters, budgets, leadership tasks such as staff supervision, completion of performance appraisals
