About the Role

Collaborate in the provision of efficient and effective outpatient and ambulatory care for patients of the state of Queensland when referred to the Kidney Supportive Care at sites throughout Metro North Hospital and Health Service at a level of a skilled practitioner appointed to this level.
Actively participate in the hospitals post graduate medical education program.
Work with multidisciplinary teams across inpatient wards, centre based dialysis units, satellite dialysis units, outpatient clinics, procedural units and educational clinics.
Participate in a rotating roster across the service line to provide continued care to all patients across facilities in Metro North.
Participate and collaborate in the planning, implementation and evaluation of the educational programs of the Kidney Health Service and Palliative and Supportive Care Service.
Demonstrate the ability to integrate knowledge of the basic sciences and results of research studies into clinical practice.
