A Temporary Full-Time Position has become available for a Patient Journey Facilitator

What You'll Be Doing

The position of Patient Journey Facilitator will have the role of improving the journey for patients through Shellharbour Hospital, from admission to discharge. This will be achieved through the change management process in collaboration with key stakeholders both internally and across the District.

The Patient Journey Facilitator works closely with the Whole of Health Lead to achieve sustained improvements in patient safety and flow. Will identify issues with patient flow and implement and evaluate projects to improve flow throughout the hospital. Will support and guide other staff with change management methodology to make improvements in service. And a ttend length of stay meetings, focusing on overcoming barriers to ‘back of house’ patient flow.

Selection Criteria

  • Current registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Broad and extensive recent clinical care experience in an acute healthcare setting with proven coordination skills in a complex environment.
  • Excellent communication skills, both written and verbal with the ability to relate to senior medical and clinical staff.
  • Demonstrated leadership and capability in implementing a person-centred model of care in a multidisciplinary team environment.
  • Broad knowledge of clinical pathways, expected length of stay and resource utilisation.
  • Demonstrated ability to work independently and as a member of an interdisciplinary team.
  • Broad knowledge of the NSW Ministry of Health Whole of Hospital Program and ability to implement sustainability strategies related to the program.
  • Demonstrated computer skills including the ability to effectively use and generate reports from the patient administration system, patient flow portal, and electronic medical record.
