JOB DESCRIPTION

Do you want to be part of an exciting, dynamic community Hospital in the Home service? You would be part of a multi-disciplinary team, providing acute and post-acute care to patients within their home environment or the APA clinic. There are two positions available, one is full time and one is 32 hours per week.

Employment Type: Permanent Full Time and Part Time roles available.
Position Classification: Registered Nurse
Remuneration: $1,233.80 - $1,732.40 per week

Where you'll be working
The position is mainly based in Mona Vale Community Health centre, Mona vale Hospital, from where you will visit patients in their homes within the NSLHD. You will also treat patients in the hospital based HiTH clinic.

What you'll be doing
Assess, plan, implement and evaluate nursing care in collaboration with individual/s and the greater health care team to achieve quality patient care outcome.
 
Candidates will need to meet the following criteria:

  1. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  2. Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).
  3. Demonstrated effective interpersonal, verbal and written communication skills.
  4. Current clinical knowledge and clinical problem solving ability.
  5. A commitment to excellence in nursing practice and continuing professional development.
  6. Ability to work effectively within a multi-disciplinary team.
  7. An understanding of the professional, ethical and legal requirements of the Registered Nurse and Holds a valid NSW driving license
