About The Role

The newly established Living Well Team's (LWT) focus is to provide case management and psychological intervention to consumers with a Personality Disorder. Intervention includes Dialectical Behaviour Therapy (DBT) and Brief Intervention following the Project Air guidelines (Gold Card Clinic). This team will also have a focus on providing consumers with the option of Virtual Care making use of state of the art technology and systems. Case Management numbers will be capped.

The Clinical Nurse is a registered nurse who is accountable for clinical mental health practice at an advanced level. This role supports the delivery of services in the areas of assessment, intervention, treatment planning and evaluation for adults with severe and enduring mental health needs, primarily with a diagnoses of Personality Disorder within a community mental health setting.

About You

Our ideal candidate will be someone who can demonstrate the following:

  • Provide direct clinical services including assessment, planning and delivery within the context of a multidisciplinary environment, maintaining an awareness of community and organisational objectives. These services include the use of core discipline assessment skills, diagnostic and formulation skills, treatment planning and case management skills, incorporating knowledge of the Mental Health Act 2016 and other relevant legislations.
  • Maintain a high level of communication with members of the multidisciplinary team, consumers, and carers and actively participating in multidisciplinary committees as directed by the Team Leader &/or Clinical Leaders.
  • Demonstrate willingness to improve clinical experience through engagement in continuing professional development training and formal qualifications.
  • Provide clinical practice supervision and mentoring to less experienced staff (where applicable) and contribute to student supervision within the specialised area of community mental health service delivery.
  • Monitor and report on clinical practices and outcomes within clinical service area and actively participate in initiating, developing and implementing quality service improvement activities consistent with organisational planning that enhance the delivery of discipline specific and multidisciplinary services within the local service area.
