We have an exciting opportunity to join Aspire4Life as a Home Support Assessor, on a full time basis. In this role, you will conduct assessments using a holistic and client centered approach, with a focus on wellness and reablement. These assessments will support older Australians to continue living independently within their chosen community, by considering needs such as assistive technology, social support services, and community care. You will have the opportunity to travel throughout the Southern Highlands to work collaboratively with clients in their homes.

This position will commence end of June on a fixed term contract until 30 June 2022 (with the possibility of renewal based on funding). This is a full time role with the inclusion of a fleet vehicle to use for both work and personal. Candidates will preferably need to reside in Batemans Bay (or between Batemans Bay and Bermagui).

This position offers a fantastic 2 weeks of full-time training and orientation at our head office in Coolangatta, QLD (all expenses covered), commencing at the end of June. You can also take advantage of other benefits, including salary and entertainment packaging and discounted gym attendance for applicable gyms.

A typical week as an assessor:

  • Travel throughout the Southern Highlands Aged Care Planning region, visiting clients in their homes as you utilizie your clinical expertise to conduct holistic style assessments, with a focus on wellness and reablement
  • Capture vital client information to build on the My Aged Care client story and develop an individualised support plan underpinned by the clients unique goals
  • Develop strategies to connect people with appropriate services and supports in their community (may it include assistive technology, social support or community care) to further empower the client with their independence

Need to be/have:

  • Knowledge of assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in aged care, community services, assessment and/or coordination of services
  • Confident communication and interpersonal skills, with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach
  • Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region

What to bring?

  • Tertiary qualification as a Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist (very highly regarded)
  • Current AHPRA Registration (or relevant)
  • Unrestricted Australian driver's licence and own reliable insured vehicle (and be comfortable travelling long distances by car)

You will be supported by our vibrant centralised liaison team who coordinate directly with the clients – you will not need to schedule appointments or worry about planning travel distances or timings! Whilst you are out on the road, your regional lead will be there to provide support and guidance along the way.
