About Aged Persons Mental Health Teams


The community teams have six sub-teams within two larger teams which are based in Kingston Centre and is known as the Middle South Aged Persons Mental Health Team (APMHT). The other is based in Endeavour Hills and is known as the Dandenong APMHT. The service provides assessment and treatment for people aged 65 years and over, who have a psychiatric disability and/or dementia with associated behaviours of concern requiring specialist treatment.


About the Role


As a member of an exciting multidisciplinary community team you will provide comprehensive clinical mental health assessment and treatment. We will look to you to deliver a range of clinical roles including triage, intake, mental state examinations, risk assessments, care planning, treatment, case management and discharge planning.


What you Need:




  • Extensive experience as a qualified Mental Health Nurse, Occupational Therapist, or Social Worker



  • Aged Persons Mental Health experience highly regarded



  • Clinical background



  • Patient Centred Care



  • Competent decision making and passion to achieve exceptional outcomes for consumers
