The Opportunity:

Due to unprecedented growth, we have an opportunity to join our friendly and passionate team as a Clinical Care Managersin Perth, WA. In this challenging role you will ensure best practice care for our clients across a multidisciplinary team with a focus on quality service delivery. To succeed you will be proactive, passionate and take pride in your work.

This role plays an integral part within myHomecare in ensuring quality service and maintaining the highest standard of care. The successful candidate will:

  • Working collaboratively with the team to provide care expertise and deliver best practice to clients living at home.
  • Acting as the key contact for clients ensuring effective outcomes and quality clinical care.
  • Manage client service delivery across all levels under the government funded CHSP and homecare packages programs.
  • Engage with internal teams to facilitate client on-boarding through the lead to conversion process.
  • Utilise your influencing and partnering skills to lead and work collaboratively with our schedulers, clinical team and support staff.
  • Ensure compliance with all relevant legislative and industry standards.
  • Proactively engage in continuous improvement across practice and service delivery.

The successful person for this role will have excellent communication and interpersonal skills and an authentic desire to support our clients and make a difference. In addition, you will be able to demonstrate:

  • Current AHPRA registration
  • Proven recent experience in case management in Aged Care or Community Services sector
  • Desire to deliver the highest standards of client service across diverse client’s groups
  • Clinical experience
  • National Police Certificate issued within the past 12 months or willingness to obtain
  • A valid C Class Driver’s Licence
