JOB DESCRIPTION

The Regional Assessment Service (RAS) is one of a broad range of human services that APM delivers, including Employment Services, Injury Management services, allied health interventions & community care services. We are committed to excellence in service delivery & looking for someone with a passion for aged care and enabling better lives.

We are looking for a full-time Team Leader to manage our team of Home Support Assessors in New South Wales. You will reside in the Western Sydney or Orana Regions.

Regional Assessment Services Team Leader

As a RAS Team Leader, you will work to deliver and oversee in-home face-to-face assessments using a standardised national assessment tool. Together with your consumers you will develop individualised support plans to achieve wellness and healthy aging goals, as well as matching and referring clients to appropriate support services.

You will train, assist and support your team to provide optimal service to our customers and deliver high quality assessments whilst also ensuring that the performance requirements for your Regions of responsibility are achieved. You will be required to build and maintain strong relationships with services providers and be rewarded for reaching monthly KPIs.

What We Need From You

  • Previous experience leading or managing a team
  • Minimum qualifications Certificate IV Community Services, Aged Care or similar
  • Experience in working in a KPI environment
  • Understanding of sales principles
  • Passion for working with aged Australians & commitment to maximise their independence & autonomy
  • The ability to communicate clearly and form effective relationships with all service providers
  • Professional presentation, flexibility, adaptability and a strong work ethic
  • Confidence in the use of mobile technology (eg tablets and mobile phones)


Highly Regarded But Not Essential
  • Experience in Aged Care assessments and case management
