What you'll be doing The purpose of the Homeless Health Outreach Team is to address the clearly demonstrated need for services for homeless people with mental illness and/or alcohol and other drugs issues. The aim of the service is to support people experiencing or at risk of homelessness to actively engage in healthcare and access mainstream services of their choice. This includes assisting people to access specialist health care and ensure they are supported in improving their health and well-being outcomes.
The Homeless Health Outreach Team will provide services to people who are chronically homeless with complex needs and diverse value systems, and who do not access standard health services. While there is a strong focus on mental health and drug and alcohol issues, general physical health issues will be within the scope of this team. This team will work very closely with the non-government services and other local and state government services that are involved in providing services to this clientele.
Undertaking the operational management of an integrated team of mental health professionals (the Homeless Health Outreach Team), providing mobile assertive clinical interventions for people in primary and secondary homelessness in the Tweed Byron areas who present with mental health and drug and alcohol issues.
Ensuring delivery of effective, responsive and clinically appropriate services within the NSW Health Performance Framework.

Selection criteria

  1. Registered Nurse with the Nursing and Midwifery Board of Australia and a minimum of 5 years’ full time equivalent post registration experience and post registration qualifications in Mental Health or area relevant to the clinical specialty. OR Tertiary qualifications and, where applicable, membership of the relevant association or registration with the AHPRA, in one of the following disciplines: Psychology, Social Work or Occupational Therapist, To be appointed as a Level 4 Health professional evidence must be provided of three years' clinical experience and demonstrated experience of extensive specialist knowledge or a high level of broad generalist knowledge within the discipline as defined by the award definitions of a level 4 practitioner.
  2. Demonstrated experience and specialised knowledge working with clients who have complex mental health needs, in particular those who are homeless or at risk of homelessness.
  3. Demonstrated knowledge and understanding of service provision for people with mental illness who are at risk of homelessness, including strategies for successful long-term accommodation.
  4. Proven leadership, team development and managerial skills, including financial management skills.
  5. Demonstrated high level communication skills, including the ability to negotiate with a wide range of stakeholders in the interests of ongoing provision of care for clients.
  6. Demonstrated skills in data management, analysis and reporting.
  7. Knowledge of or an ability to acquire knowledge of procedures, policies, regulations and standards which impact upon the position and a knowledge of contemporary human resource management issues such as workplace health and safety, equal opportunity, and anti-discrimination.
  8. Valid unrestricted drivers’ licence for use in NSW/Australia and willingness to travel in the course of employment.
