Community Dementia Nurse
Eastlakes Community Health Service

We are seeking a compassionate, skilled Registered Nurse to join our team of Community Dementia Nursing Team to provide support to the client living with Dementia in the community. The Community Dementia Nurses form part of Aged Care Services and they operate out of a number of Community Health Centres in the district.

About The Opportunity

This position will see you providing safe compassionate person centred care for the client living with dementia in the community in collaboration with client / families/carers and the multidisciplinary team to achieve identified goals and outcomes. Upon commencement you will receive an extensive orientation and will be further supported by the skilled team.

About You

The successful applicant will be a motivated, hardworking professional who is passionate about working with clients living with dementia. You will be experienced in caring for the client living with dementia, possess great communication and problem solving skills. Your advanced computer literacy will be key for writing care plans and reports.
