About The Role

As a Home Support Assessor you will provide basic home based face to face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF). Working with each client, you will establish goals specific to the client; develop appropriate strategies and actions, including referral to aged care and non –aged care specific services; and develop a Support Plan. You will complete the NSAF and match and refer clients to appropriate services. Initial training in all systems is provided prior to conducting face to face assessments including facilitated completion of a nationally accredited Statement of Attainment in Aged Care – Home Support Assessor. HNELHD Regional Assessment Service also has a dedicated training officer for support as needed.

About The Service

The Northern HNELHD Regional Assessment Service provides in-home, Aged Care assessments to eligible My Aged Care client’s throughout the New England and Lower Mid North Coast regions. There are 10 staff members strategically placed throughout the Region, the position being recruited is primarily based in the Tamworth Local Government Area however there are rare occasions where travel to other areas may be required. All training is completed within the team and in collaboration with the Ministry of Health and Department of Health for consistency and alignment with the national Aged Care Framework.

About You

You are an excellent communicator with an ability to relate to aged care clients objectively and compassionately, assessing their needs and concerns and developing a support plan to ensure ongoing independence and safety in the home. You have knowledge of the aged care sector, My Aged Care and community based services coupled with medium to high level computing skills (Microsoft Word, mobile computing etc.) highly regarded.
