About the role

Eastern Health is collaborating with St Vincent’s Hospital Melbourne (SVHM) to provide the Healthy Ageing Initiative to consumers aged 65+ years (or 55+ years for Aboriginal and Torres Strait Islander people) who are at risk of or diagnosed with mild to moderate mental illness  who live independently in the community or in Residential Aged Care Facilities (RACFs).   

The Healthy Ageing Service Response (HARS) provides:

  • Secondary consultation for General Practitioners (GPs) and their practice staff, and RACF staff, supporting older persons with mental health challenges living independently in the community or in Residential Aged Care Facilities (RACFs)
  • Primary consultation and/or brief intervention support with the older person and their GP/RACFs team, at the clinic or RACF where clinically indicated, using mental health clinicians and psychiatrists. 
  • Sector capacity building General Practitioners (GPs) and their practice staff, and RACF staff, across the EMPHN catchment, to increase confidence in assessments, support older people with mental health and wellbeing issues, promote team-based care and collaboration to provide whole-of-person care.

The Consumer and Carer workforce is an integral component of the Mental Health Program and includes a range of Lived Experience Workforce roles.  The HASR Consumer/Carer Lived Experience Peer Support role has been developed specifically for people with lived experience (consumer and carer) of mental health issues and mental health service use and will work as part of the multidisciplinary Healthy Ageing Service Response team contributing specific knowledge, skills and expertise.  The successful applicant will preferably be Aboriginal or Torres Strait Islander as this role will partly focus on supporting people aged 55+, and their families, who identify as Aboriginal or Torres Strait Islander, to help engage in their own mental health care and treatment.  The role will also focus on service development and promotion of this new service. The Consumer/Carer Lived Experience Support Worker will work in collaboration with the Mental Health Social and Emotional Wellbeing Team, Aboriginal Lived Experience Peer Support Worker, Expanding Post Discharge Team and the Eastern Health Aboriginal Health Liaison Officer, the Aboriginal Health Team.

About you 

To be successful in this role you will possess:

  • Lived experience as a consumer or family carer of a person who has used public mental health services.
  • Understanding of mental health/social emotional wellbeing peer support principles and ability to use peer support to provide hope and a belief in recovery.
  • Understanding and appreciation of the principles of recovery and self-care using peer based examples of wellness.
  • Understanding strengths based model of recovery.
  • Demonstrated understanding of and empathy regarding the broad impact of mental illness on consumers and carers/families.
  • Demonstrated ability to apply cultural knowledge within the model of recovery to support consumers and carers.
  • Desire to support Aboriginal people to engage with Aboriginal Controlled Organisations and mainstream health services.
  • Ability to work within a multidisciplinary team including ability to express own viewpoint and respect the views of others.
  • Understanding of the need to be sensitive to the needs of people from diverse backgrounds including Aboriginal and Torres Strait Islander people, Culturally and Linguistically Diverse people
  • Contribute to staff training on understanding and being sensitive to the needs of Aboriginal and Torres Strait Islander people
  • Effective communication skills.
  • Ability to manage time and resources effectively.
  • Ability to work in a challenging environment.
  • Basic proficiency in computer skills.
  • A satisfactory Police Check is required prior to appointment.
