Social Work Services

The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting. It is a requirement of this role to participate in the on-call rosters across Monash Health.

About the Role

The Social Work Renal program is looking for an experienced Grade 2 social work clinician to join the team across Nephrology servicing Casey, Clayton, Cranbourne, Dandenong and Moorabbin. This is a temporary part-time role (0.8 EFT) to March 2022. Previous chronic illness and hospital experience is essential. This is an interesting and dynamic role where you will have the opportunity to work in satellite dialysis, home haemodialysis, peritoneal dialysis and support of the acute nephrology inpatient ward. The position is primarily based at Thomas St Dandenong site but work across the spectrum of nephrology may be required.

This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans including patients and families facing life-threatening situations and decision making. The diversity of the role requires clinical knowledge that ensures a safe, high quality Social Work service for all clients.

Deliver and drive Monash Health strategic intent to be relentless in our pursuit of excellence across Social Work and Allied Health.

Work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios.

Travel is required across Monash Health sites.

About You

You will be a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing Nephrology patients and their families with a sound understanding of pathways and referrals networks. You must be eligible for AASW membership including continuing professional education

Proven experience in providing social work in acute and subacute settings.

Knowledge and experience working closely with inter-professional teams.

Current Driver’s Licence.
