About the Role
This will be achieved through liaison and networking with key stakeholders both internal to Primary Care and Community Health (PC&CH) and the Nepean Blue Mountains Local Health District (NBMLHD) and externally with Nepean Blue Mountains Medicare Local (NBMML), GPs and the broader community sector. The aim of this role is to facilitate integration of systems to inform and coordinate care by strengthening effective working relationships, partnerships and referral networks to progress facilitated multidisciplinary care for clients with chronic conditions who have complex care needs.

This position is located at Lemongrove Community Health Centre.

Selection Criteria to be addressed:

  1. Current Registered Nurse Division 1 National Registration (AHPRA) with a minimum of three (3) years full time equivalent post registration experience
  2. Post registration qualifications relevant to performing the role of Clinical Specialist Grade 2 for Chronic Care Integration Coordinator e.g. Advanced Nursing and/or Public Health or equivalent
  3. Demonstrated skills and knowledge in case management, including care coordination and an ability to promote self management for people living with complex and chronic conditions, using advanced clinical skills in patient/client assessment and care planning
  4. Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Health Policies related to Chronic Disease Management, accompanied by a knowledge of General Practice
  5. Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non government and community organisations
  6. Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based services
  7. Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications
  8. Current Driver's Licence with a proven good driving record and willingness to use for work purpose travel
