About us

headspace provides support to young people aged 12-25 experiencing mild to moderate or emerging mental health issues. The multidisciplinary team work to improve health, social and vocational/educational outcomes for young people.

Grand Pacific Health is the lead agency for headspace Canberra. We are a not-for-profit, primary health care organisation providing locally-tailored, high quality services. Services focus on physical health, mental health, youth health, Aboriginal health & chronic disease management. Grand Pacific Health covers a large portion of South Eastern NSW, the A.C.T and Murrumbidgee region. Our vision is equitable health care where everyone has access to affordable, quality and culturally appropriate health services.

About the position

The Youth Care Coordinator will work collaboratively with a young person and their family/carers to identify goals and develop collaborative care plans. The position will provide short term care coordination, deliver early intervention counselling and support services and work closely with the other members of the headspace Canberra team and external service providers to coordinate support needs.

This position is part time (30h hours per week), fixed term until 30 June 2023 with the possibility for extension. The part time roster (hours and days) is negotiable.

What you’ll need

Passion and experience working with young people
Qualifications in a relevant field or working towards same e.g. youth work, community welfare, social work, psychology, peer work, occupational therapy, counselling or other related qualifications or demonstrated experience in a similar role
Experience delivering individual and group intervention
Experience providing coordination of care and the ability to work from a recovery oriented framework
