Monash Children’s at Home, through home nursing visits, education and support to parents and carers delivered under a range of relevant programs provides healthcare services for infants, children and their families at home and in the community, who require ongoing specialised and often complex medical and nursing care.


We are now looking to engage a highly motivated professional to join our dedicated team in a role that will see you working across a rotating seven day roster.


About The Role


Reporting to the Nurse Unit Manager, this role works largely independently, providing a safe, effective alternative model of care for Paediatric patients needing ongoing nursing management at home. In this way, patients spend less time in the acute setting and we concurrently uphold our commitment to patient centred care.


Working closely with patients’ families, hospital based medical, nursing and allied health staff and community agencies, you will ensure that patients enjoy a safe transition from the acute hospital setting to the home environment. Specific responsibilities include:



  • Planning, evaluating and maintaining a high standard of safe and effective care for children

  • Conducting comprehensive health assessments and educating those involved in care planning, direct care and discharge planning in the ambulatory setting

  • Ensuring timely discharge planning to facilitate patients’ safe and effective transition from hospital to home

  • Demonstrating your commitment to developing, promoting and progressing continuous quality improvement initiatives

  • Role modelling excellence in safe nursing practice consistent with our expectations of leaders across Monash Health


This is a tempoary fixed term part-time .8 EFT position. 


About You 


You are passionate about patietn and paediatric care. Utiliting your previous paediatric clinical nursing experience working within Monash Children’s at Home, or a similar role, you have strong relationship building skills and are outcome focussed in this highly satisfying role enhancing families’ independence. This role will enable you to also:-



  • Apply your professional nursing skills in this alternative care model

  • Demonstrate your commitment to patient centred care

  • Ensure the delivery of best practice treatment protocols


What You Need


To be considered for this role you will need to demonstrate that you satisfy the following mandatory criteria:



  • Current AHPRA registration and current Acute Paediatric Nursing experience

  • Demonstrated accountability for providing high quality nursing care

  • Proven ability to apply excellent clinical and critical thinking skills while working independently but within a Team framework

  • Excellent communication, interpersonal and documentation skills

  • Current Driver’s Licence and access to a reliable vehicle


You are a dedicated nursing professional who can demonstrate initiative and sound judgement in both decision-making and work planning. Previous experience in community nursing, will be highly regarded.
