About The Role

The Enrolled Nurse/FIM coordinator practices under the direction and supervision of the Nursing Unit Manager- Patient Flow and in accordance with NSW Health guidelines and local policy.

The role will be accountable for delivering, maintaining and assigning a system-wide FIM assessment and documentation for all Rehabilitation patients at Calvary Hospital. The position will ensure accurate scoring of FIM items at admission and discharge, in conjunction with the Australasian Rehabilitation Outcome Centre (AROC) guidelines and identify strategies on how the data can improve future service efficiency.

The Enrolled Nurse/FIM coordinator will also collaborate with the Multi- Disciplinary Team and play an active role in case conference, by supporting the team members in using the AN-SNAP Calculator tool and the timely completion of required documentation.

The position will be pivotal in the delivery of planning the patients Rehabilitation journey, uploading documentation and referrals on the Rehab database, ensuring accurate FIM changes, in line with length of stay (LOS) outcomes and identifying potential quality improvement initiatives, whilst supporting the bed manager as required.
