A professional development opportunity exists for an experienced, reliable and motivated Registered Nurse who is interested in enhancing their skills undertaking a CNS role. The Clinical Nurse Specialist provides holistic, specialist palliative care to clients of the Community Palliative Care Team (CPCT) both in the home and clinic environments.

The CNS2 is responsible for the coordination and continuity of care for palliative care patients utilising a consultative process to optimise clinical practice and patient management.

About The Role

The CNS 2 is responsible for assessment and coordination of patient referrals, and planning, advising, directing and evaluating the care of palliative care patients linked to the Palliative Care service at Calvary Health Care Kogarah (CHCK). The key responsibilities include liaising with Palliative Care Specialists, Nurse Unit Manager (NUM) and other staff within and outside CHCK to facilitate appropriate access to CPCT and inpatient Palliative Care services at CHCK, and ensure the practice of Palliative Care nursing is safe and efficient, maintaining standards of care through continual quality improvement initiatives.
