About the role

An opportunity exists for a motivated individual to join the Ambulatory Aged Care Programs (AACP) as a Social Worker. The Social Worker is responsible for the planning and implementation of high quality Social Work interventions.

We are looking for

An approved tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
Well-developed time management and problem-solving skills
The ability to prioritise tasks and work under minimal supervision
Excellent written and verbal communication and interpersonal skills and be able to work effectively within a team environment

Essential : Demonstrated understanding of person centred care in relation to adjustment to illness, disability or lifestyle changes
Essential : Demonstrated ability to work independently and as active member of an interdisciplinary team
Essential : Excellent interpersonal, written and verbal communication skills, including the ability to initiate and maintain effective professional relationships with a wide range of internal departments, community agencies and services
Essential : Demonstrated time management skills and organisational skills required to manage a clinical caseload and non- clinical responsibilities as required
Essential : A satisfactory National Criminal History Record Check, Victorian 'Employee' Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration will be required from prospective candidates prior to commencement of employment. Desirable : Possess an understanding of VCAT, guardianship and administration procedures
