About the role

The Aged Psychiatric Assessment & Treatment Team is seeking a mental health case manager to join our team and work in the Shepparton area. The Case Managers role is to work productively with all team members to provide a high standard of psychiatric assessment and case management for aged consumers living in the community. The Case Manager is responsible for providing assessment and treatment of aged clients who have a chronic or acute mental health illness in the community. The position requires a broad based experience, knowledge and expertise in aged mental health with a thorough understanding of current treatment approaches and practice.

We are looking for

AHPRA Registration as either a Registered Nurse, Psychologist or Occupational Therapist or eligibility for membeship of relevant allied health professional association
Demonstrated competence in community based mental health assessments and case management
Applicants must have a sound knowledge and understanding of the Victorian Mental Health Act 2014
Well developed time management & problem solving skills
Demonstrated knowledge of the planning, implementation, and evaluation of a range of individual and group interventions for people with mental health problems.

Selection Criteria

Desirable : Understanding of the Victorian 2014 Mental Health Act
Desirable : Broad clinical experience and/or discipline expert knowledge with at least 2 years post graduate experience working in mental health
Essential : Current Registration with Australian Health Practitioner Regulation Agency (AHPRA) and/or eligibility for membership of relevant allied health professional association
Desirable : Experience dealing with clients from culturally and linguistically diverse communities.
Essential : Highly effective communication and interpersonal skills, including the ability to relate positively with and appropriately with patients and others
Essential : A satisfactory National Criminal History Record Check, Victorian 'Employee' Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration will be required from prospective candidates prior to commencement of employment.
