The Team

We are looking for an experienced Registered Nurse who is able to work autonomously. Someone who is confident and capable and wants to excel in a new area of growth. RMH@Home subacute is an innovative inpatient bed substitution program that is modelled on a rehabilitation and Geriatric Evaluation and Management (GEM) patients. RMH@Home subacute provides a seven day specialist medical, nursing and allied health service to assist patients to rehabilitate within the comfort of their own home.

The aim of this service is to provide delivery of patient centred safe and effective care in the home environment. As a Road Nurse your role will be assessing patients at home, liaising with families and our coordinator. You will be required to liaise with the medical and allied health team and foster a cohesive team environment.
