The opportunity

Working as part of a broad multi-service, multidisciplinary healthcare organisation, you’ll lead a team of nurses to coordinate and support the delivery of Hospital Care at Home services to clients across Australia. Your management of clinical care will be driven by a human-centred, holistic and evidence-based approach.

What you’ll be doing

Leading a team of nurses and supporting staff to coordinate quality Hospital Care at Home programs for clients transitioning from hospital to home
Developing and supporting staff to build an engaged and skilled team of care coordinators
Building productive relationships with referring hospitals, funders of programs and community services providers
Optimising systems, processes and workforce management to effectively and efficiently manage referrals and service throughput
Supporting a customer service culture through sustained actions and commitment to improve client and referrer experience
Helping to improve the health and wellbeing of hundreds of Australians.

Your skills and experience

Register Nurse with current AHPRA registration
Experience in care coordination and/or acute nursing in the community
Excellent knowledge of hospital discharge planning
Experience in contributing to continuous quality and process improvement
People leadership and management experience desirable
Understanding of the private healthcare setting desirable
Customer focus, initiates action, tech savvy, coaches and develops others
Passionate about health and committed to making a real difference in the community.
