About the team

Come and join the GV Health Rural Allied Health Team (RAHT), a team with a well renowned supportive workplace culture with an opportunity to work alongside experienced Allied Health Professionals. You will work autonomously, undertake a varied and diverse workload and provide interventions in predominately home based settings, making a true difference to our client’s independence and well-being. There is no nursing home work.

The team is an established and progressive multi-disciplinary team which includes Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work and Speech Pathology services, supported by Allied Health Assistants.

About the role

The Social Worker will provide person centred care with an opportunity to:-

Travel across the Goulburn Valley region working in home based settings.
Work with clients, carers and families to provide support, counselling, information, advocacy advice and referral with a focus on informed decision making and empowerments solutions to promote independence home and in the community.
Develop goals and care plans to assist clients to remain at home and active in their local community.
Advise on strategies and areas of assistance to support clients to remain living independently at home and within their local communities.
Work with a diverse range of clients and health conditions.
Undertake teaching and training opportunities with local service providers.
Participate in student training programs.

We are looking for

Bachelor of Social Work or Master of Social Work (qualifying) and a member of the AASW.
Broad knowledge in the planning, implementation and evaluation of current Social Work practice within health care settings including home based and community settings.
Experience in the delivery of Social Work services to clients with complex presentations, inclusive of mental health problems, complex health conditions, cultural and diverse backgrounds and social disadvantage.

Selection Criteria
Essential : Commitment to the GV Health Values of Compassion, Respect, Excellence, Accountability, Teamwork and Ethical Behaviour and ability to exhibit behaviour which reflects our values.
Essential : Australian Association of Social Work (AASW) accredited Bachelor of Social Work or Master of Social Work (qualifying) and membership of the AASW.

Essential : Experience in the delivery of Social Work services to clients with complex presentations, inclusive of mental health problems, complex health conditions, cultural and diverse backgrounds and social disadvantage.
Essential : Experience in providing services within a self management framework and active service model/reablement approach to care delivery.
Essential : An excellent level of knowledge of the local service system, and ability to liaise and advocate across the full spectrum of health and welfare services.
Essential : Excellent interpersonal, written and communication skills, including computer literacy and the ability to produce well-written reports.
Essential : The ability to work collaboratively as an individual practitioner within an interdisciplinary team environment including the ability to initiate and maintain effective professional relationships.
Essential : Proven organisational and time management skills with an ability to prioritise and manage a diverse caseload.
Essential : A satisfactory National Criminal History Record Check, Victorian 'Employee' Working with Children Check and completion of a Commonwealth of Australia Statutory Declaration will be required from prospective candidates prior to commencement of employment. Do you agree to completing these requirements?
Desirable : Post-graduate qualifications in gerontology are advantageous, but not essential.
Desirable : Experience working with frail older people, people with disabilities and their carers, and people living in rural communities.
Desirable : An ability to liaise with a broad range of regional service providers, and members of the community.
Essential : A Clear / Satisfactory result from the National Disability Insurance Scheme Worker Screening Check.
