The Aboriginal Health Worker - YouthHealth provides flexible, holistic and culturally safe health to

Aboriginal and Torres Strait Island clientsand the community, to achieve better health outcomes and better access tohealth services for Aboriginal and Torres Strait Island young people, 12-24years of age, and their families/carers in South Western Sydney. The positionwill support community development and health promotion initiatives that engageyoung Aboriginal and Torres Strait Island people.

Based at the Youth Health Service, thisposition works within a dedicated multidisciplinary team to deliver highquality care in an environment, providing health promotion and healtheducation, intake assessment, and case management. The incumbent will workindependently and/or under the supervision and direction of a healthprofessional and within the limits of the Aboriginal Health Worker positiondescription and qualification. What you'll be doing

The Male Aboriginal Health Worker-Youth Health position is responsiblefor delivering culturally responsive, flexible and holistic primary health carefor clients of South Western Sydney Local Health district. The role will entailservice delivery across the LHD, from designated service locations: The Corner(Bankstown), Traxside, (Campbelltown), Flyht (Carramar) and outreach services,under limited supervision, as required.

The aim of the role is to achieve optimal health outcomes and facilitateaccess to health services for Aboriginal & Torres Strait Islander youngpeople, 12-24 years of age, and their families/carers in South Western Sydney.The position will support community development and health promotioninitiatives that engage young Aboriginal & Torres Strait Islander people.
