Alfred Health delivers a comprehensive stroke service with acute care delivered on The Alfred hospital site and specialist rehabilitation and community services based at the nearby Caulfield Hospital site. As well as directly admitting over 500 acute stroke patients a year, The Alfred provides tertiary stroke services for endovascular and neurosurgical care for surrounding hospitals. There is a 24/7 thrombolysis and endovascular clot retrieval service.


This is a full-time fixed term post and part of Alfred Health’s commitment in delivering excellence in clinical neuroscience care. The Stroke Coordinator is responsible for facilitating coordinated care for Stroke patients presenting directly or as tertiary referrals to Alfred Health. This includes the provision of services through the Emergency Department, acute inpatient wards, transfer to subacute/rehabilitation facilities and discharge into the community. It is a position for a person with a passion for stroke and patient centred care. The stroke co-ordinator will work closely with the Director of Stroke Services, the ward service stroke consultant, the nurse unit managers of 7 West (stroke) and 6 East (neurosurgery) wards, the stroke nurse educator and the stroke data manager as well as the hospital bed management team.

Key Duties And Responsibilities
  • Provide a single point of contact for users of the Alfred health stroke pathway
  • Coordination, monitoring and evaluation of efficient and effective delivery of stroke care throughout the health service from acute, subacute and discharge planning within the interdisciplinary team.
  • Ensure the provision of and coordination of patient focused outcomes and implementation of evidence based stroke care
  • Coordinating and promoting multidisciplinary family meetings for stroke patients where appropriate
  • Lead the assessment, planning , and evaluation of stroke care ensuring compliance with best practice to achieve a coordinated patient journey from admission through to community reintegration
  • Develop, review and implement relevant pathways of care and/or clinical guidelines/policy, to ensure that care across the continuum, is consistent, effective, efficient, evidence based and equitably accessible to all patient groups
  • Actively participate in stroke services activities
  • Organise and facilitate stroke patient and family education by overseeing implementation of ‘My stroke journey’
  • Work with Director of Stroke and Clinical Service Director to implement Stroke models of care and monitor compliance and clinical outcomes i.e. Hyperacute Stroke Unit Care, Code Stroke
  • Work across Stroke and Neurosurgery to manage all patients admitted with Intracerebral Haemorrhage.


  • Professional Registration with relevant AHPRA Board
  • At least 5 years clinical experience in Stroke Care
  • Relevant Clinical Master’s – Advanced Practice
  • Be able to demonstrate strong communication, interpersonal and organizational skills (including time management)
