The Position

Position Title: ACAS Clinician
Classification: According to EBA
Hours per fortnight: 64
Employment Status: Fixed Term-Part Time


Aged Care Assessment Services (ACAS) provide comprehensive assessments, using a multi-disciplinary and multi-dimensional approach, to determine eligibility for Commonwealth government subsidised aged care services and support care coordination. ACAS operates throughout 9 Local Government Areas in the Loddon Mallee Region.


Key Responsibilities:

  • Work autonomously in the community undertaking comprehensive, holistic assessments and participate in review and management, where indicated, for eligible clients.
  • In consultation with the client, their family, local service providers and other team members, formulate a suitable care plan encompassing all aspects of the client’s continuing care that includes a choice of available services and referral options.

Key Requirements:

  • University or equivalent qualification in nursing or allied health equivalent with current registration with, and/or professional membership of, relevant professional body as required
  • Demonstrated commitment to working in a multidisciplinary team recognising and respecting the contribution of all team members

Selection Criteria
Essential:
01.University or equivalent qualification in nursing or allied health equivalent with current registration with, and/or professional membership of, relevant professional body as required

Desirable:
02.Demonstrated understanding of health and related issues for elderly people, people with disabilities, and their carers

Desirable:
03.Experience in and demonstrated understanding of aged care services including rehabilitation and community services

Desirable:
04.Demonstrated commitment to working in a multidisciplinary team recognising and respecting the contribution of all team members

Desirable:
05.Demonstrated ability to communicate effectively across client, community and professional groups inclusive of skills to negotiate and manage conflicts which arise

06.Flexibility to operate in an environment of change and continuous improvement

Desirable:
07.Demonstrated high level computer skills including word processing, data entry, emails and an ability to adapt to changing ICT demands. Use of mobile devices in the field is required
