JOB DESCRIPTION

SYC is now looking for a Specialist Support Coordinator who will promote choice and independence for eligible participants of the National Disability Insurance Scheme (NDIS) through the provision of professional advice and linkage to service providers.

This full-time position is based in Mount Gambier.

Duties of the successful applicant will include:

  • Work to achieve required outcomes specified in NDIS funding agreements and guidelines.
  • Interpret client NDIS Plans and where applicable, manage services against budgets.
  • Conduct assessments to establish the participant’s personal goals and barriers.
  • Deliver person-centred support within SYC’s Case Management model and the NDIS Code of Conduct, promoting independent decision-making, choice and control.
  • Coordinate participant supports by liaising with a range of allied health specialists, community support agencies and services, and make referrals where appropriate.
  • Support participants to achieve their goals by building their capacity to coordinate their own supports.
  • Advocate on behalf of participants with relevant services and Government departments and attend appointments where necessary.
  • Work closely and effectively with external service providers, especially the National Disability Insurance Agency (NDIA), NDIS assessment services, and NDIS service partners to ensure that services complement each other.

Qualifications and Experience:

  • Minimum bachelor’s degree in Social Work, Psychology, Occupational Therapy or other relevant qualification is essential.
  • Allied health registration is essential.
  • Experience in engaging and supporting people with complex support needs is essential.
  • Experience and knowledge of the NDIS and/or the disability sector is highly desirable.
  • Experience in achieving outcomes with minimal supervision and direction.
  • Experience in conducting client assessments, developing case plans and case notes and report writing.
  • Experience with building and maintaining relationships with community organisations, employment services, training and education bodies and Centrelink systems; and able to provide advice regarding accessing these services and benefits.
