The Role

An exciting opportunity has arisen to become a member of our high achieving, client centred team with the Transition Care (TCP)/GEM@ Resi program. The program is seeking a dynamic and experienced health professional from an allied health (Grade 2) or nursing background (Division 1) to fulfil the role as TCP/GEM@Resi Care Coordinator. This is a Part Time position at 48 hours per fortnight

Transition Care provides case management and care coordination from hospital to community care settings, and this role can encompass all streams of Transition Care (Home based and Bed Based) and GEM@Resi.

The Transition Care/GEM@ Resi Team is a friendly and dedicated team who support each other to provide optimal care for aged clients following a hospital stay. Working as part of a multidisciplinary team is required. Supervision and professional development opportunities are provided.

To Be Successful In This Role You Will Have

  • Experience and sound working knowledge of community aged and disability care programs
  • Experience in completing geriatric comprehensive assessments and care plan development
  • Proven ability to work in both the hospital and community settings
  • Excellent communications skills, including capacity to negotiate effectively.
  • The ability to work in a fast paced environment and establish priorities
