The Role

An exciting opportunity has arisen to become a member of our high achieving, client centred team with the Transition Care (TCP)/GEM@ Resi program. The program is seeking a dynamic and experienced Allied Health Assistant Grade 3 to join the team. This is a Part Time position at 48 hours per fortnight.

Transition Care provides care coordination and low level therapy from hospital to community care settings, and this role can encompass all streams of Transition Care (Home based and Bed based) and Gem@Resi.

The Transition Care / Gem@Resi team is a friendly and dedicated team who support each other to provide optimal care for aged clients following a hospital stay. Working as part of a multidisciplinary team is required. Supervision and professional development opportunities are provided.

To Be Successful In This Role You Will Have

  • Experience working in Allied Health (physiotherapy, occupational therapy) in aged care
  • Proven ability to work within a team environment
  • Demonstrated ability to follow instructions
  • Proven equipment ordering and maintenance experience
  • An understanding of the roles and contributions of difference disciplines to client care and discharge planning.
