About the ED Mental Health & AOD Hub

The Monash Medical Centre Emergency Department has an interprofessional team which brings together clinicians from mental health, alcohol and other drug (AOD) and the ED to provide specialised and integrated care to patients presenting to the ED with high acuity mental health and AOD issues, and complex psychosocial needs. This service is called the Mental Health and AOD Hub and is located in the emergency department. The team provides a seven day a week, extended hours service.

The team seeks to develop positive relationships with people in crisis, and deliver care and support that respects the individual’s rights, preserves their dignity, and promotes recovery and inclusion.

About the Role

This unique senior social worker role presents a fantastic opportunity to join a new team in the ED implementing an enhanced model of care. The social work role seeks to provide person centred care that encourages and supports patient and carer participation, facilitates safe and timely discharge from the Emergency Department to the community and promotes continuity of care and linkages across hospital and community services. The SW3 works in partnership and collaboration with other members of the interdisciplinary ED Hub team, mental health program, AOD services, allied health and lived experience workforce.

  • Provide high level leadership, clinical advice and advocacy for patients presenting in crisis with mental health, AOD and/or psychosocial needs.

  • Perform specialist social work assessment, interventions and short term assertive outreach support to individuals who have presented to the Clayton ED and been identified as a patient with complex psychosocial needs.

  • Work collaboratively and effectively with multiple stakeholders (including NOK, carers, family and/or nominated persons, multidisciplinary team and community agencies).

About You

You are an experienced social worker who holds a qualification that provides eligibility for membership with the Australian Association of Social Workers (AASW). You are able to demonstrate specialist mental health assessment skills and have experience working in acute mental health settings and/or the emergency department. You possess excellent communication skills and have proven leadership qualities. You have a passion for improving the experience of care for people in crisis and delivering services that respect the individual's rights, preserves their dignity, and promotes recovery and inclusion.
