About The Role

The WMHHS Indigenous Hospital Liaison Service provides support and assistance to Aboriginal and Torres Strait Islander patients, families and Community across all facilities in the West Moreton Hospital and Health Service. The Indigenous Hospital Liaison Officers work collaboratively with multidisciplinary teams within the West Moreton Hospital and Health Service to ensure that culturally appropriate, sensitive services contribute to the improved health and wellbeing of Aboriginal and Torres Strait Islander people. The WMHHS Indigenous Liaison Service is a key component to the Integrated Care High Performance Care System model. The model provides case management, care navigation and health coaching to support patients to access appropriate services across multiple care providers and to better navigate the health system. The coordinator will have the responsibility for the oversight and development of the Indigenous Hospital Liaison Service across the West Moreton Hospital and Health Service.

To perform this role, it essential that the person who holds the position be an Aboriginal or Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 the applications are Aboriginal or Torres Strait Islander.

  • identifies as an Aboriginal and/or Torres Islander person; and
  • is of Aboriginal and/or Torres Islander descent; and
  • is accepted as an Aboriginal and/or Torres Strait Islander person by the Aboriginal and/or Torres Strait Islander community in which he or she lives.
About You

Our ideal candidate will be someone who can demonstrate the following:
  • Ability to combine your lived experience of Aboriginal and Torres Strait Islander family structures, culture, practices and protocols with advanced management skills and advanced mental health and/or addictions knowledge and understanding of historical practices to deliver culturally responsive services.
  • Proven ability to lead, develop and manage cultural services by using your cultural expertise, community connections and leadership skills to establish strong relationships between Aboriginal and Torres Strait Islander peoples within West Moreton Health.
  • Demonstrated ability to work effectively with Aboriginal and Torres Strait Islander people (including patients, carers and communities) within a health-related context with a clear understanding of Aboriginal and Torres Strait Islander culture, protocols and health related policies.
  • Highly developed written and verbal communication, interpersonal skills and a demonstrated ability to communicate effectively within a multidisciplinary clinical team.
  • Demonstrated ability to actively participate in a working environment supporting quality human resource management practices including employment equity, anti-discrimination, occupational health and safety and ethical behaviour.
