The Role

An exciting opportunity exists for an enthusiastic Social Worker to join the team at Northern Health.

This role requires a Grade 2 Social Worker to provide comprehensive psychosocial assessments, intervention and discharge planning to patients of Northern Health based at our Geriatric Evaluation & Management (GEM) unit at Kilmore and District Hospital.

To Be Successful In This Role You Will Have

  • The Grade 2 Social Worker will have a minimum of 2 years experiences, providing quality patient care through the application of competent clinical skills in assessment and plan a variety of suitable interventions.
  • Experience and good working knowledge of My aged care services, the Victorian Civil and Administration Tribunal (VCAT) process and the National Disability Insurance Service pathway.
  • Proven ability to work independently and as part of a multidisciplinary team
  • A commitment to self initiated continuing professional development
  • Dedication to non-clinical programs such as staff supervision, student education and quality / innovation.
