About The Role

Working within a primary health care framework and using a client centred strengths based approach the CNS2 Community Rapid Access Inreach will provide a conduit to facilitate information sharing, planned care and care coordination for identified clients across the community – hospital – community continuum through liaison and networking with key stakeholders both internal to Community Health Care and Nepean Hospital and externally with General Practitioners and the broader community sector.

Selection Criteria To Be Addressed

  • Current Registered Nurse Division 1 National Registration (AHPRA) with a minimum of three (3) years full time equivalent post registration experience in a specialty area of practice related to the management of clients with chronic conditions who have complex care needs and a related recognised post graduate clinical qualification (Graduate Certificate level or equivalent).
  • Demonstrated advanced clinical skills and contemporary knowledge of evidence based Chronic Care Nursing practice and NSW Ministry of Health policies and initiatives for Chronic & Complex Care.
  • Demonstrated evidence of being able to provide person centred care and an ability to liaise with other health professionals to facilitate care navigation and care coordination.
  • Demonstrated ability to work independently with minimal supervision whilst maintaining involvement within the wider multidisciplinary team and with other service providers including Hospitals, GPs, Community Services and NGOs.
  • Demonstrated problem solving and decision making abilities and high level communication, liaison, negotiation and time management and report writing skills.
  • Demonstrated ability to provide leadership in the provision and development of clinical practice and a demonstrated commitment to own professional development.
  • Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications and commitment to undertake training in electronic health record systems.
  • Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
