What You'll Be Doing

The primary purpose of this new Fracture Liaison Coordinator role is to undertake the implementation and coordination of the SWSLHD Osteoporosis Re-Fracture Prevention (ORP) Service. The Fracture Liaison Coordinator is involved in the identification, assessment, care coordination and case management of patients attending the SWSLHD ORP Service, under the clinical oversight of a Medical Officer.

This role will work in close collaboration with the NSW Agency for Clinical Innovation (ACI) Musculoskeletal Network. The Fracture Liaison Coordinator will follow the overarching principles of the ORP Model of Care as described by the ACI, with consideration given to local resources and requirements.

Provide safe, compassionate, quality, person centred specialist care and work in collaboration with patients/families/carers and a multidisciplinary team to achieve identified goals and outcomes.

How To Apply

To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

  • Demonstrated extensive clinical experience and skills in providing complex care in the relevant field.
  • Demonstrated understanding of effective models of chronic disease management and experience in relevant service delivery.
  • Demonstrated working knowledge of the contemporary issues around Osteoporosis and its management.
  • Demonstrated skills in developing collaborative working relationship and ability to work both within a multidisciplinary team and independently.
  • Demonstrated high-level effective communication with the multidisciplinary team, internal and external key stakeholders and patients/carers to achieve patient and project outcomes
  • Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidence based practice.
  • Demonstrated experience in risk management, work health and safety and quality improvement in the workplace
  • Demonstrated proficiency in a range of information technology systems pertaining to health care including Microsoft Office systems and effective time management skills.
