The Role

An exciting opportunity exists for an enthusiastic Grade 2 Occupational Therapist to join the team at Northern Health in our HARP program at Broadmeadows Hospital. This is a full time, fixed term role until June 30th 2022.

HARP aims to assist clients with chronic diseases and complex care needs to optimize their health outcomes and to reduce preventable use of emergency departments and inpatient services. HARP provides timely comprehensive assessment, care-planning, service coordination and short-term intervention to clients who are at risk of readmission to hospital or presentation to Emergency.

As part of this multidisciplinary team the occupational therapist provides client-centred specialist input to clients requiring OT assessment/intervention e.g. for provision of aids to support mobility/function or pressure care.

To Be Successful In This Role You Will Have

  • Experience and good working knowledge of complex health care needs of community dwelling individuals.
  • Proven ability to work autonomously and part of a wider multi-disciplinary team
  • Outstanding customer service skills
  • Sound Occupational Therapy skills in assessment, intervention and discharge planning
  • High level of problem solving capabilities
