Western Health Medical@Home are presenting an opportunity for SMS with an interest in developing new models of care alternative patient pathways within the @Home model of care to join the team.

The Medical Model for the Western Health HITH Unit was set up in 2015 in order to provide better medical governance for patients admitted under HITH and to facilitate improved patient flow within the organisation. We are now expanding this role to cover non-admitted patients in the community at high risk of hospital re-admission.

The Medical team running the @Home Unit will comprise of 2 streams @Home – Non-Admitted and @Home –Admitted and staff will rotate between these services. Both streams will have a dedicated HMO and Senior Medical Registrar.

Our services have expanded significantly over the past six years and we have embarked on new endeavours continuously in order to meet novel demands of care in the community (e.g. heart failure treatment, COVID+ve patients monitoring).

We provide timely assessments and clinical reviews of non-admitted and admitted @Home patients and facilitate outpatient follow up of recently discharged @Home patients.

Our consultants also provide clinical support to our HMO’s and Registrars and our @Home Nursing and Allied Health teams, in hours and after hours, to ensure a high quality of patient care.

A maximum of 14 hours will be appointed to an individual applicant.

To be Successful in this Role you

  • Will have experience across a wide range of medical disciplines

  • Will have experience working with patients with multi morbidities within your current specialty

  • Will preferably have cardiology and / or endocrinology experience

  • Will demonstrate excellent communication skills and experience in working within multidisciplinary teams and environments

  • Will have experience in successfully working and interacting with patients across a wide variety of cultures and socio economic circumstances
