  • Grade 2 – Subacute, Angliss Hospital

  • Part Time Position (24 Hours Per Week)

About the role


This position provides an exciting opportunity for you to work as part of a subacute multidisciplinary team and develop your skills in assessment, discharge planning and collaborative team work. You will experience a supportive environment that ensures structured supervision, professional development opportunities and engagement in research and quality activities.


As the successful applicant you will have competency in psychosocial assessments, risk assessments, grief and bereavement support, residential processes, care planning, decision making and discharge planning, as well as liaison with internal and external key stakeholders. You will also assist in the development of services within the clinical stream by applying evidence based and client centred care within a multidisciplinary setting.


With a 'can do' attitude, flexibility, resilience, and excellent interpersonal skills, you will be a valued member of the Eastern Health Social Work team.


You will be responsible for:



  • Providing a clinical social work service to subacute patients at Eastern Health

  • Conducting comprehensive clinical assessments and discharge planning with patients 


Key Selection Criteria 


Essential



  • Eligibility for membership of the Australian Association of Social Workers

  • Demonstrated capacity to work effectively in a fast-paced health environment

  • Demonstrated expertise in conducting risk assessments

  • Demonstrated expertise in complex discharge planning

  • Demonstrated capacity to practice autonomously

  • Demonstrated expertise in working in a multidisciplinary team

  • Demonstrated understanding/experience in social work student supervision and/or teaching

  • Excellent communication skills

  • Current Continuing Professional Development (CPD) requirements in line with AASW CPD standards


Desirable



  • Previous health care social work experience

  • A current Victorian driver's licence
