JOB DESCRIPTION

  • Small home care package provider - report directly into the Managing Director
  • Coordinate services following a client directed model of care
  • AHPRA registration is not required!

Your next employer:
Be a key team member at a locally owned, small provider of home care packages. They believe in supporting clients to be at home, allowing them to live their life, their way and pride themselves on offering individually tailored services including assistance with everyday tasks, social support, home maintenance and respite care.

The exciting opportunity:
  • Based in Perth’s Southern suburbs, with most of their clients in the Fremantle area
  • Report directly into the Managing Director at an organisation with 105 clients on home care packages
  • Responsible for the home care coordination of community clients, promoting healthy ageing and increasing their ability to stay in their home
  • Liaise with ACAT Assessors and conduct initial visits with potential new clients
  • Create, review and update care plans applying a focus to a person centred care model that empowers the elderly to meet goals and maintain their independence
  • Ensure suitable support services are provided that will improve quality of life and facilitate community re-engagement
  • Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements
  • Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options
  • Provide leadership, support and mentoring to the support staff
  • Be involved in identifying gaps and creating continuous improvement initiatives

About you:
  • Ideally you will hold AHPRA registration as a Registered Nurse but this is not mandatory
  • Experience in a leadership role within residential aged care or home care is essential
  • Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
  • Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
  • A current driver’s license and the ability to manage your time and work independently
