The Role:

An exciting opportunity exists for an enthusiastic Registered Nurse, Social Worker or Allied Health Professional to join the team at Northern Health.

Northern Health Home Care Packages offers Case Management packages to older adults with complex health and social needs living in the community. The NH HCP program includes Linkages packages and Commonwealth Home Care packages.

This role requires someone who will demonstrate a consumer centred, collaborative approach to their work, who has highly developed clinical skills and is capable of managing diverse and sometimes challenging situations.

NH HCP offers a strong sense of teamwork, rewarding clinical experiences and opportunities for further professional development. The successful applicant will work within a multi-disciplinary team and have access to regular supervision and support.

Please note: There is a possibility of contract extension due to program growth or other short-term employment in other Community Services at Northern Health, such as TCP in the future.

To be successful in this role you will have:

  • Experience and good working knowledge of Consumer Direct Care within Home Care Packages.
  • Proven ability to work within a person-centered model of care.
  • Professionals who have experience in aged care, community care, case management or care co-ordination are encouraged to apply.
  • The ability to speak a second language would be an advantage.
  • A sound understanding of “risk vs rights” within a community setting.
  • Knowledge and understanding of the structure of individualised budgets within Consumer Directed Care.
