JOB DESCRIPTION

Will you actively create a healthier future for tomorrow?

At Medibank we're encouraged to think big. We have a clear purpose to impact better health outcomes for our customers, patients and our community.

Our Story:

Home Support Services (HSS) which is part of the Medibank Enterprise, has been providing in-home healthcare services since 1989. We understand that Australians want more choice over where and when they receive care. Where clinically appropriate, our national home healthcare business, Home Support Services (HSS), offer people the flexibility of receiving healthcare and support services in alternative settings to hospitals, wherever they call ‘home'.

Your Opportunity

Reporting directly to the Clinical Services manager you will draw on your extensive experience as a Palliative Care Registered Nurse to deliver best practice holistic clinical palliative care within the community. This role will give you the opportunity to work autonomously as well as coordinating a collaborative team whilst working closely with our Palliative Care Specialist to develop this new program. We are open to applications from candidates seeking both fulltime and part time work.

This is a pivotal role in the development of the clinical program. An initial 12-month contract will be offered with a view to extend. You will be;

  • Coordinating all aspects of patient care from assessment and care planning to providing direct patient care
  • Liaising with hospitals, doctors and family members to provide safe palliative care to patients in their own home
  • Managing the logistics of palliative home care services, including medication and stock requirements

What will set you apart from the rest?

  • Current unrestricted AHPRA registration as a Registered nurse
  • Palliative care qualifications along with minimum 5 years' experience in a similar role
  • Clinical knowledge, skills & experience in acute care
  • An expert in providing patient centred, evidence-based specialist services for patients with palliative and end of life care needs
  • You will be naturally empathetic, observant, attentive, trustworthy, and committed to making your patient feel safe and comfortable
  • Exceptional communication and organisation skills

We value the knowledge and contribution of Aboriginal and Torres Strait Islanders and are working hard to create an inclusive workplace and develop Indigenous careers. We're also committed to supporting better accessibility for our people. If we call you to discuss this role, you'll also get the opportunity to chat about any adjustments that will allow you to be your best in the role.
