Senior Social Worker - Acute Psychiatric Unit of the Adult Mental Health Division

This position is located Acute Psychiatric Unit. The 19 bed Acute Psychiatric Unit provides inpatient services to people over 18 years of age who have an acute psychiatric disorder requiring specialist intervention for assessment and treatment. The Unit has a strong integration with mental health community-based services supporting continuity of recovery focused care. The Unit also provides five inpatient beds for services for consumers with an eating disorder, who require an acute health restoration program due to medical risk because of the severity of their eating problems. An additional six beds are located adjacent to the Unit for the Parent-Infant program.

The teams within NEAMHS are made up of highly passionate, experienced, supportive, collaborative and enthusiastic clinicians! A thorough working knowledge of the Mental Health Act (2014), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines Demonstrated knowledge of recovery principles you will have mandatory qualification as a social worker and have at least 7 years experience to be able to apply for this senior role.

The primary focus of this role will work

  • In conjunction with the SW2, and another senior social worker this role will provide direct services to patients and their families/carers/significant others aiming to minimise the negative impacts of illness and hospitalisation and enhance social and emotional functioning through targeted discipline specific social work interventions including the mobilisation of services and supports.

  • The SW3 has a critical role in assessing patient’s social environments and relationships, recognising the effects of the psychological, familial, social, economic and cultural determinants on health and wellbeing, advocating for the rights of patients and their families/carers, assisting patient flow through coordinating and collaboratively developing discharge plans and post discharge follow up.

  • Duties will include engagement and assessment of patients on the Unit in conjunction with the multidisciplinary team. While the role will maintain a focus on discharge planning and identification of need for service post discharge, the SW3 will concurrently undertake psychosocial assessments, and undertake family meetings on the ward. The eligible patients will only be those who reside in the NEAMHS catchment area

  • This senior social worker role will take the lead in coordinating family assessments for all patients and their families on the acute unit. The social worker will prioritise patients and their families who are attending the APU for the first time and ensure a coordinated approach occurs as part of this admission. As part of the assessment process the senior social worker will determine the number of family sessions required and will coordinate the appropriate membership of these meetings.

  • The membership of the family meetings may include the registrar, consultant, occupational therapist, psychologist and nurse.

The social worker may consider the following:

  • Psychosocial assessment

  • Psychoeducation

  • Family work

  • Family therapy session

  • Establish post discharge supports for the consumers

  • Establish post discharge supports for the family

  • Ensure appropriate linkages are undertaken for children under the age of 18 years

  • This team has as its focus effective discharge planning and support for patients and families in community post discharge with the goal of enhancing engagement and reducing risk of relapse and readmission.

  • To enable flexibility for families to attend family assessments and sessions the social worker will need to be able to provide evening sessions and be available to work on a Saturday. 

  • This role will working closely with the Community Transition Coordinator and these roles will be colocated at APU on the weekends and evenings.

While the SW3 is operationally accountable to the APU Nurse Unit Manager the senior social worker will work closely with both the Transition Coordinator role at APU and the Clinical Nurse Consultant role in relation to discharge planning and coordination of external services.
