About the Role:

With the expansion of the company, we are looking for a Care Coordinator who is a Division 1 Registered Nurse with aged care experience. This managerial role is suitable for a person who enjoys human interactions, building new relationships, managing staff members and passionate for creating a difference in others’ lives. Previous management experience is desirable but not essential.

The Care Coordinator is responsible for:

  • Leading the care team in providing support to consumers for them to remain living independently within their homes and community
  • Visiting the clients at their homes and undertaking comprehensive assessments; identifying their goals and needs; producing care plans and the corresponding service schedules
  • Ensuring delivery of exceptional quality of care by monitoring, training, and supervising care team
  • Advancing service quality by contributing to continuous improvement initiatives
  • Actively building relationships and promoting of the services of the company
  • Contribution to staff recruitment and retention

Essential key selection criteria:

  • Division 1 Registered Nurse with current AHPRA registration
  • Current First Aid and CPR certificate
  • Recent clear National Police Check
  • Excellent communication and networking skills
  • Well-developed planning and time management skills
  • Strong knowledge of ageing and disability concepts
  • Having a registered and fully insured vehicle
  • Current full driver’s licence

 Candidates should not have any history of indictable offence, bankruptcy, any sanctions on their licence and current or previous association with any other home care agency.


This is a flexible casual position with an opportunity to become an ongoing full or part time position after commencement for the right candidate. Work hours are flexible and suitable for retired or semi-retired registered nurses wishing to maintain work-life balance while continuing to create a difference in the community. A mixture of working from home, working in the office and client visits will be required.

A negotiable generous hourly rate is offered with additional bonuses and benefits being available for the right candidate.
