An exciting opportunity exists for a skilled hospital Social Worker with a commitment to patient centred care to join the very experienced and dedicated Hornsby Ku-ring-gai Hospital Social Work Department.

Where you'll be working
Hornsby Ku-ring-gai Hospital is located in the suburb of Hornsby. The hospital is currently undergoing significant redevelopment and the Social Work Department offers a supportive team environment.
What you'll be doing

- Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
- Proven ability to work independently and to establish effectively collaboratively working relationships within a multi-disciplinary team and interagency networks.
- Demonstrated well developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders.
- Commitment to participation in professional development and quality improvement activities.
- Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning.
- Demonstrated ability to prioritise and manage workload demands, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required).
- Demonstrated computer literacy and ability to use health information systems.
