An exciting opportunity exists for a skilled hospital Social Worker with a commitment to patient centred care to join the very experienced and dedicated Hornsby Ku-ring-gai Hospital Social Work Department.

Where you'll be working
Hornsby Ku-ring-gai Hospital is located in the suburb of Hornsby. The hospital is currently undergoing significant redevelopment and the Social Work Department offers a supportive team environment.

What you'll be doing

As a part time Social Worker at Hornsby Ku-ring-gai Hospital, you will be working as a member of the multidisciplinary inpatient team providing Social Work clinical interventions whilst partnering with patients, families and carers. 
Candidates will need to meet the following criteria:

  1. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
  2. Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
  3. Proven ability to work independently and to establish effectively collaboratively working relationships within a multi-disciplinary team and interagency networks.
  4. Demonstrated well developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders.
  5. Commitment to participation in professional development and quality improvement activities.
  6. Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning.
  7. Demonstrated ability to prioritise and manage workload demands, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required).
  8. Demonstrated computer literacy and ability to use health information systems.
