What You Will Be Doing

We have an exciting opportunity for a Case Manager to join our Community Mental Health Team. The Frankston Adult Case Management Team provides community treatment through recovery focused case management for clients 25-64 years Monday through to Friday (8:30am-5pm).

In this role you will be providing timely and responsive mental health assessments and treatment to people who require community based mental health treatment and support, assisting clients to achieve their optimal health outcomes and empowering the individual with the belief that they can and do have control over their lives.

What You Need

We anticipate you will have:
•Current registration in one of the following; Registered Nurse (with a post graduate qualification in psychiatric/mental health nursing or specialized undergraduate psychiatric mental health nursing program), Psychologist (with relevant National Board clinical psychology for Psychologist membership), Occupational Therapist (with Occupational Therapy Board membership) or Social Worker (with a degree in social work that is able to be approved for membership of the Australian Association of Social Workers)
•Knowledge and understanding of the Victorian Mental Health Act 2014, the National Standards for Mental Health Services 2010, the National Standards for the Mental Health Workforce 2002 and the Australian Council on Healthcare Standards including EQuIP
• Recent experience in mental health (including acute inpatient services and/or community mental health care
