This an exciting opportunity for a motivated and experienced candidate to join the Aged and Disability Services team at Maroondah City Council. This position is an integral part of the My Aged Care Regional Assessment Service Outlet supporting referred older people to access services to assist them to remain independent in their own home and community.

You will:

have demonstrated experience in assessment, ideally including direct experience as a My Aged Care Home Support Assessor;
hold a tertiary human services qualification such as social work, allied health or related;
have a work history that demonstrates a strong commitment to listening to client’s and connecting them to support that is important to their needs, with a focus on enhancing client choice, control, independence, wellness and reablement;
enjoy the benefits of working in a multi-disciplinary team and meeting a broad range of people in the community;
have a sound understanding of State and Federal aged care policies, systems and supports, as well as an ability to network, build partnerships and work independently and collaboratively.

You will be part of the broader Community Services department and will be based at Ringwood, however will extensively travel across Maroondah, reporting to the Coordinator Assessment. Please address all selection criteria in your application.
